K - 12

Students will receive the following screenings

with referrals as necessary

 K - 12 - vision, height & weight

K - 3 - BMI (Body Mass Index)

K - 3, 7 & 11 - hearing screening

Grade 7 - scoliosis screening

Kindergarten Entrance Requirements

• DPT - 3 doses (4th one on or after fourth birthday)

• Polio - 3 doses

• Hepatitis B- 3 doses

• MMR - 2 doses

• Varicella - 1 dose (or proof of chicken pox disease)

Physical Exam -by  school physician or private physician

Dental Exam - by school dentist or private dentist

No child will be permitted to enter school

without a copy of immunizations on file in nurse's office

Grade 3  Requirements

Dental exam- by school dentist or private dentist

Grade  6 Requirements

Physical exam - by school physician or private physician

Grade seven Requirements

Immunization requirements

Hepatitis B - 3 doses

Varicella - one dose or proof of chickenpox disease

Dental Exam - by school dentist or private dentist

Grade 11 Requirements

Physical exam - by school physician or private physician
